- CV/Resume: Please limit the resume to two pages and use the eHealth Resume Template
- Unofficial Transcripts
Unofficial transcripts for all post-secondary study at a university level. Upload a scanned copy. Original/official transcripts and language testing results will only be required if applicants are accepted into the program. - References
Two references including at least one academic reference is preferred. Enter the name, institution, and email address for each referee. Referees will receive an email containing a link to provide their reference. Professional references may be used if applicants have graduated 5+ years ago – in the reference section, please select: Professional in lieu of Academic Reference (graduated 5+ years ago). For exceptions please contact the Program Administrator. - IELTS or TOEFL Results
- Thesis Stream Only: Research Statement
Provide a one page Research Statement that includes:- summary of your research accomplishments
- a potential research problem in eHealth that you are interested in studying and
- brief details of a possible research method you could use to address the problem

Application Fee
Applications must be accompanied by the required $150 application fee.
- This fee is non-refundable and must be paid in Canadian dollars.
- If there is an issue with payment, the application will not submit.
- When payment is approved, applicants will receive an application acknowledgement email confirming that the application has been submitted successfully.
What Comes Next
Our admission committee reviews each completed application that meets the minimum requirements noted on the Admissions tab.
If you are applying for one of the full-time options, a face-to-face interview is the final step of the application process. The interview helps determine how suitable you are for an 8-month internship, which is a required component of the full-time program.
